Card Delivery *The Greener Institute is providing our patients with a streamlined process that expedites the delivery of the cards.Due to COVID-19, I understand it is now an average of 5-14 working days to receive my card. Since The Greener Institute has no control of mailing, I understand that the card may take up to 21 working days or longer in rare cases. *The card will be shipped to the address on my drivers license unless I supply a change of address to The Greener Institute before I see the doctor.Appointment Time *Even though I have selected a specific appointment time on the calendar, the doctor works within a window of time. I will be added to the list for the Doctor to contact me on my appointment day between the hours of 9-3pm. The doctor will NOT call me. He will send a TEXT. *The text will contain a link that I will click on to start the video conference.State Account *All patients doing a price match must create their own state account using their drivers license at https://padohmmp.custhelp.com prior to their doctors appointment. Once you are certified by the doctor, the State will send the following 2 emails:The 1st email will be from the state informing me that I have been approved by the doctor to get my mmj card, and I should go into my state account and pay $50. *The 2nd email will be a receipt for the $50 dollar state fee I paid. If I do not receive this email 24 hours after my doctor's appointment, I will contact The Greener Institute.Renewal *I understand that if I am applying for a recertification and have not used The Greener Institute initially to get my certification, I am responsible for going into my state account and paying the state the $50 fee.Refund policy *I will receive a 100% refund if the doctor does not certify me. *If I decide to cancel my appointment after my deposit has been made, I will receive 40% of my payment back.Digital Signature *Date * By typing your name, you are signing this Agreement electronically.
